- Job description**Post Ref**: CC268- Please note: Priority applicants will be seen in the first instance.- A Shared Lives Officer Supports the Registered Manager within a CQC regulated setting to ensure high quality provision of services to older people, people with a learning disability including young people from 16 years of age and people with physical and mental health needs to a high standard of care consistent with individual needs in accordance with statute and Government Guidance.- Effectively manage a caseload of shared lives placements supporting people with critical, substantial and often complex needs and provide clear supervision, direction, guidance and support to a team of allocated Shared Lives Providers.

We are proud to work collaboratively.- To continue the development of this service, the Shared Lives team are looking to recruit a Shared Lives officer to support with the delivery and monitoring of our service.- A good communicator verbally and Non
- verbally.- Be able to work with people from diverse backgrounds. Work collaboratively with our partners and in a person -centred way with the people who use our service.- Be able to negotiate and consult with others.- Be able to undertake assessments, formulate and present action plans that progress the person
- centred work we do.- Have an ability to work on their own initiative using their own judgement and work to a high level of accuracy meeting statutory regulations. With the ability to implement specific policies and procedures to meet CQC standards.- Have a sound knowledge and understanding of confidentiality and data protection.- Have previous experience of caseload management.- Be able to work flexibly on occasions working outside of normal working hours to respond to emergencies offering excellent service for the people we support.- Travel for work purposes.- Work with people within a social care setting.
