Due to new funding, this job is an exciting opportunity to support delivery of a Value-Based Lymphoedema Healthcare Service for Wales. The post holder will support the National Lymphoedema Team in providing an evidence based Lymphoedema assessment and treatment for the people with Lymphoedema in Wales.

This post will clinically support the new ‘On the Ground Community Lymphoedema Educator Teams’, providing a proactive approach to lymphoedema/ chronic oedema management. They will also work with the award winning ‘cellulitis Improvement Programme’ across all Health Boards in Wales and commence a new project with Heart Failure patients who have chronic oedema.

The post will work autonomously within professional guidelines and exercise responsibility for the governance of their patients.

The post holder will be involved in assessing and promotion of the accredited Agored Cymru Lymphoedema/ Chronic Oedema education courses.

The role will involve facilitating and coordinating a portfolio of quality improvement work including implementation of the ‘Wet Leg Pathway’, collecting Patient Reported Outcome Measures, and driving person-centred approaches to care in lymphoedema/chronic oedema; wound care, compression and cellulitis assessment/ treatments for adults.

The post holder will be expected to travel around Wales although the funding and management is allocated to Swansea Bay UHB the base can be different depending on applicant’s location.

Education will be an integral part of this role as well as involvement in research, audit and evaluation. This will include implementation of the ‘Wet Leg Pathway’, collecting Patient Reported Outcome Measures, and driving person-centred approaches to care.

The post holder will work closely with other members of the National Lymphoedema Team providing high quality specialist and standardised lymphoedema/chronic oedema; wound care, compression and cellulitis assessment/ treatments for adults.

The post holder will be expected to travel around Wales although the funding and management is allocated to Swansea Bay UHB the base can be different depending on applicant’s location.

The ability to speak Welsh and to have a driving license is desirable for this role.
