This role is based in Colchester and the main **duties** includeprocessing referrals received either direct or through a wide range of agencies, working within hospital discharge and many other stakeholders (this list is not exhaustive) to identify the most appropriate service, club or support to meet the needs of the individuals that offer practical, social and emotional support that will improve their health and wellbeing.
You will build relationships with the people you are helping by listening carefully to what is important to them and what motivates them. You will work individuals, their families and carers to take control of their wellbeing, live independently and improve their health outcomes.

This role would suit someone who has the resilience to deal with complex and sometimes challenging situations.
**What you can expect**:
- A base in Colchester but there needs to be flexibility with working location (Hospital etc)
- 28 days holiday (including bank holidays)
- An extra day’s holiday for your **birthday **(after passing probation)
- **Five extra days** holiday (pro-rata) after five years’ service
- Up to **three** days volunteering per year (after passing probation)
