Primary Care Networks are collaborations between groups of practices to develop services to improve patient care and reduce health inequalities.

Healthwest PCN is a mature PCN in the heart of Bristol, comprising Pembroke Road Surgery, The Family Practice, The Student Health Service and Whiteladies Health Centre.

You will work as part of our small and friendly prescribing hub team, providing cost effective best practice in prescribing to improve the quality of patient care. You will provide an efficient and safe system of medication reviews, liaising with all relevant parties, including phone call contact with patients and other health care professionals regarding medication queries. Ensuring all administration duties and patient requests are processed in accordance with practice policy.

Full training and on-going support will be provided by our pharmacy team. You will need to be an excellent communicator and be passionate about helping to improve the quality of patient care.

**Roles and responsibilities**

Process repeat prescriptions on a daily basis ensuring compliance with the prescribing hub protocols.

Ensure all current medication have relevant, current authorisations.

Promote cost-effective prescribing at the practice through switching drugs to the generic preparation when appropriate and encouraging the use of local primary care trust formulary medication choices.

Deal with prescription queries from a range of healthcare professionals including; GP’s, nurses, community pharmacists, nursing home staff as well as reception/administration staff at the practice.

Add new patient medication. Pass to registered GP for approval highlighting any concerns found, add relevant diary entries/recall. Liaise with patient via phone call, SMS or letter. Liaise with patient’s pharmacy regarding changes, if necessary.

Liaise with Pharmacies regarding monthly dosette box prescriptions.

Contact patients regarding relevant health checks e.g. BMI, BP, Cholesterol et.

Attend regular prescribing hub team meetings and practice meetings, as necessary.

Any other duties as reasonably requested.

Evidence of a sound education

Relevant knowledge of Primary Care Networks, General Practice and the wider Health and Social Care policies would be helpful

**Skills and Competencies**

Computer literate with an ability to use EMIS and Microsoft office packages **(desirable)**

Has attention to detail, able to work accurately

Has a planned and organised approach with an ability to prioritise their own workload to meet strict deadlines

Excellent communication skills, verbal and written, with the ability to adjust communication style and content to suit the audience
