We are looking to recruit a Therapy Services Coordinator to be part of our Therapy Coordination Team.

Therapy services coordinators are responsible for processing all the referrals which are received by CancerCare and you will liaise with assessors, therapists and the wider team to ensure that clients receive the most appropriate service for them, in a timely way.

You will be based at our Slynedales Centre in Lancaster although occasional travel to centres in Kendal, Barrow and Morecambe will be required.

**Main Duties and Responsibilities**
- When required, undertake initial assessments either at CancerCare’s premises or (very occasionally) in people’s own homes to supplement the assessments carried out by our referrals’ assessor
- Be available to clients who walk in and need to talk and/or be assessed immediately
- Provide information on all aspects of client services at CancerCare to clients, enquirers and referrers
- Co-ordinate the referral and assessment process
- Track availability in each therapy and allocate referrals appropriately
- Record client deaths and bereavements and notify therapists involved
- Management of confidential client records (digital systems, hard copy, current and deceased)
- Respond appropriately to the need to cancel or re-allocate clients in the event of their therapist’s absence
- Arrange relief cover for our hospital work, where possible, during therapists’ absence
- Allocate referrals for assessment in line with processes and protocols
- Allocate approved assessments to the appropriate therapist in line with processes and protocols
- Liaise with therapy staff to manage continuity of care for clients
- Provide information to therapy staff as appropriate
- Liaise with referrers and related agencies in line with processes and protocols
- Liaise with other CancerCare staff in particular support service staff to ensure clients are at the ‘centre of support’ given by CancerCare
- Liaise with referrers, GPs and clients during therapy and on discharge as required

**Job Type**: Part-time
Part-time hours: 22.5 per week

**Salary**: £12,858.00 per year
