An exciting opportunity has arisen for a Social Care Coordinator in our Hambleton Central Community Social Care team. The team is based at White Rose House, Northallerton and the team covers the Market Town of Northallerton however we do operate a hybrid working approach.

**The Role**

This is a 6 months post.

The core focus of this job is to maximise the independence and safety of vulnerable adults of North Yorkshire who have social care needs.

The post holder will do this by undertaking assessments of need with vulnerable adults, using the Directorate's procedures and where required, will undertake assessments of need of the vulnerable adult’s carer(s).

You will support adults and carers who have been assessed as meeting the eligibility criteria under the Care Act 2014, to develop creative support plans and provide appropriate care packages to meet their preferred outcomes within specified resources.

You will signpost those who do not meet the eligibility criteria, to other relevant prevention services and community-based support.

You will be able to respond quickly to urgent and changing situations, to minimise risk to an individual’s safety and wellbeing; whilst respecting the person’s choices or acting in their Best Interests - and be involved in identifying and raising concerns through safeguarding.

You will have a good understanding of the Care Act 2014 and the Mental Capacity Act 2005 and the key principles underpinning the legislation.

You will be able to put this knowledge into practice or willing to take responsibility for your development in this area, where training and guidance will be provided.

You will demonstrate the importance of dignity, respect, and professionalism in all aspects of the role.

You will work closely in partnership with a variety of teams and agencies, including our health colleagues, stronger communities, housing, prevention teams and independent sector organisations.

You will be required to maintain clear, concise and timely records on case files (both electronic and paper) in line with Directorate policy.

The ability to manage your time effectively, use a computer with confidence and good written and verbal communication skills are essential.

There is an expectation that the successful applicant will be able to travel across the locality and have a full UK driving licence, a vehicle for work use and hold business insurance.

Previous experience of working within either the adult Health or Social Care sector is essential. An NVQ 3/level 3 Diploma in Health and Social Care (or equivalent) is also essential.
