You will work within the Richmondshire PCN which covers the following practices: Scorton Medical Centre, Scorton; Quakers Lane Surgery, Richmond; The Friary Surgery, Richmond; Doctors Lane Surgery, Aldbrough St John; Catterick Village Surgery, Catterick; Leyburn Medical Practice, Leyburn; Harewood Medical Practice, Catterick Garrison and Central Dales Practice, Hawes. The successful applicant will be joining a dedicated multidisciplinary workforce delivering a personalised and integrated care service for all registered patients. They will become part of a forward thinking PCN which is committed to the development of a dedicated, caring and resilient multidisciplinary primary care workforce. Key Responsibilities Take referrals from a wide range of agencies ie.

GP practices, pharmacies, MDTs, hospital discharge teams and voluntary, community and social enterprise (VCSE) organisations and provide a personalised package of support to individuals as appropriate. Provide personalised support to individuals, their families and carers to take control of their wellbeing, live independently and improve their health outcomes. Develop trusting relationships by giving people time and focus on what matters to me. Develop good working relationships with Primary Care colleagues within the PCN.

To promote social prescribing, build relationships and work with GPs to develop their awareness on how to identify people who would benefit from the service. Working in partnership with the practice team, support patients and promote positive lifestyle changes and prevent ill-health by means of behavioural change techniques. Build and maintain effective place-based partnerships with voluntary sector providers and statutory services to support social prescribing options for vulnerable people. Provide empowering support to individuals in a planned and structured way by co-producing personalised care plans to improve health and wellbeing outcomes.

Help people identify the wider issues that impact on their health and wellbeing such as debt, poor housing, unemployment, loneliness and caring responsibilities. Help people gain or maintain independence. Keep accurate client records and collate outcomes and output data as required. Meet people on a one-to-one basis, making home visits where appropriate within organisational policies and procedures.

Provide referral agencies with regular updates about social prescribing, including training for their staff and how to access information to encourage appropriate referrals. Support local partners and commissioners to develop new groups and services where needed, through small grants for community groups, micro-commissioning and development support. Encourage people, their families and carers to provide feedback and to share their stories about the impact of social prescribing on their lives. Work with your line manager to undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities.
