Northallerton, United Kingdom North Yorkshire County Council Full time

**Grade I**

**Vacancy Hours 37**

**Contract Type: Established**

**Weekend Working: Yes**

**Interview Date: 25th August**

**Venue for Interview: White Rose House, Thurston Road, Northallerton.**

We are currently looking for
two Social Care Coordinator to help us realise our ambition of ensuring that
all residents of North Yorkshire live longer, healthier, independent lives.

We are looking for two enthusiastic full time, permanent Social Care Coordinators
to join our Discharge Hub Team in the Hambleton and Richmond Locality area. In
your role as a Social Care Coordinator, you will be working with a supportive,
likeminded Team who work alongside NHS Trust Discharge Facilitators and other
members of the multi-disciplinary team at various hospital sites.

We work together to assist in facilitating the safe and timely discharges of people from hospital. Our Team is based in Northallerton and
predominantly covers hospital discharges emanating from the South Tees Hospital
Trust sites including James Cook, Friarage, and the Friary. We do also support
Darlington Memorial Hospital, Bishop Auckland General Hospital and other
outlying hospitals too with people who are being discharged from hospital
living in the northern patch of North Yorkshire.

Working in a fast-paced environment, you will work closely with our health
colleagues to ensure that seamless pathways of care are delivered under
discharge to assess arrangements. You will undertake assessments under
discharge to assess arrangements whilst being mindful of the importance of a
strengths-based focus and preventative measures to ensure we have the person’s
wellbeing at the heart of what we do.

You may need to be involved in
considering the Mental Capacity of people being discharged from hospital, if
this is called into question and requires Best Interests decisions to be made
on their behalf involving both families, health professionals and other
relevant partners in our wider communities. We can be creative with the
approaches that we use to ensuring that someone is able to have their needs met
in a safe manner, whilst clearly noting the importance of thinking ‘Home
First’.

At the same time, you will be considering whether there are any informal carers who
may need to be referred for assessments in their own right. Within your work,
you will also have key responsibilities for ensuring that vulnerable adults are
safe from harm but where harm or abuse is identified for ensuring relevant
safeguarding enquiries are progressed.

We know that working in Adult Social Care can be both a rewarding and challenging
profession. Therefore, it’s important to us that our team members have
manageable workloads, supportive and transparent management and the opportunity
for continuous professional development.

We strive and excel in giving our colleagues commitment and high levels of
support, enabling them to become skilled in their role. We are committed to
creating a healthy culture where our team members feel able to share their
views, to achieve work/life balance and to continually learn and develop new
ways of working.

As an organisation, we promote a safe culture of support and learning which we
expect all staff to endorse. We recognise the importance of the health and
wellbeing of our employees, striving to ensure our staff have manageable workloads and providing them with specialised administrative support, allowing
focus on delivery of care services.

In addition to this we can also offer a range of benefits including generous
leave entitlement, flexible working, a local government pension scheme, salary
sacrifice and employee discount benefits. North Yorkshire is a modern council
rolling out new technology including modern IT equipment, WIFI access across
NYC buildings, remote access and Teams - all enabling and supporting
flexible working.

An Enhanced Level DBS Disclosure is required for appointment to this
post. If you would like to have an informal discussion about the role prior to
applying, please contact Paula Atkins, Team Manager on 07971 543819.
