**Pre-admission**: To ensure that admission referral forms are completed correctly and to advise referrer accordingly if further information is required. Collate all relevant information and send referral to the Multi-disciplinary team for review Maintain the referrals spread sheet and follow up on enquiries. Manage the admissions pipeline and schedule assessments efficiently. Maintain effective communication with hospitals, healthcare providers, and families.

**Admissions**: To Co-ordinate all appropriate procedures working with the (MDT) and outside agencies ensuring that all plans are in place for the time of admission. Work closely with outside agencies on arranging admissions, booking transport and liaising with the patient, the family and relevant professionals. This includes informing patients and families of visiting hours and taking note of their contact numbers. Communicate with patients, their families and the person who referred the patient.

To verify all parties understand admission plans, dealing with their concerns in a compassionate and effective manner. To play a proactive role on each admission, introducing themselves and their role to all patients and carers as well as ensuring all new clients receive a welcome letter and pack. Ensure patients and their families are offered refreshments, orientated to the CRC, shown how to use call bells etc. This will require the post holder to involve Volunteers by sharing information and knowledge so they can actively contribute to these activities.

**Discharges**: Co-ordinate, monitor and initiate appropriate systems along the client pathways to ensure the clients journey is managed efficiently, smoothly and in accordance with governance, targets and agreed timescales. Facilitate safe and timely discharge by co-ordinating all appropriate procedures working in partnership with the MDT and outside agencies, ensuring that all plans are in place for the time of discharge including transport arrangements. Establish, develop and maintain professional working relationships with referrers, clients, families, colleagues and other health and social care professionals. Build and maintain a list of suitable placement options for varied clients in the Surrey and wider areas, e.g.

supported living, behavioural units and alternative rehabilitation centres. Locate and secure an allocated social worker, if appropriate, working with them to source placement options Facilitate, attend and minute discharge meetings with clients, families and external agencies. Facilitate the completion of reports and onward referrals, working closely with the team to ensure reports and onward referrals are sent off promptly. Assist with other aspects of care such as verifying that all arrangements and equipment needed are in place for nursing and home care.

Liaise with discharge destination if residential, care homes or supportive living to arrange for assessments and identify bed availability. Liaise with the Early Supported Discharge Team (ESD), community rehabilitation and reablement teams with regard to discharge planning, home visits and estimated discharge dates In liaison with the management team, to stand in for elements of the Medical Administrators role in their absence, including attending ward rounds and taking key points for action. Attend staff meetings/Multi-Disciplinary Team (MDT) and any other meetings requested by Manager, take minutes.
