**What is the role?**

As a Creating Connections Community Support Worker at Guild Care, you will play a vital role in supporting older people to access a wide range of activities and services in the local community. Working under the guidance of the Service Manager, you will be responsible for providing practical assistance, individual befriending, and telephone support to promote community engagement and alleviate social isolation among older individuals in Worthing.
- Assisting the Service Manager, you will be responsible for:_
- Providing short-term practical support to individuals accessing the Creating Connections Service.
- Offering one-to-one befriending support and companionship to those in need of emotional assistance.
- Delivering telephone/non-contact support to alleviate loneliness and social isolation.
- Using motivational interviewing techniques to boost confidence and encourage community engagement.
- Providing additional support to individuals requiring assistance to achieve personal outcomes.
- Encouraging individuals to express their views and make informed choices about their lives.
- Raising awareness of available services, supporting access to relevant support, advice, and information.
- Maintaining a database of individuals receiving support, documenting service benefits and outcomes.
- Building and maintaining effective relationships with customers, relatives, carers, internal colleagues, and other professionals.
- Ensuring all feedback is encouraged, recorded, reported, and used to enhance the service.
- Understanding and adhering to Guild Care policies and procedures.

**What kind of person are we looking for?**
- Ability to communicate effectively with various individuals.
- Experience in engaging with older people.
- Works well in a team and individually.
- Maintains confidentiality and follows correct policies and procedures.
- Ability to travel within the Worthing area.
- Flexible availability, including occasional evenings and weekends.
- Satisfactory DBS check.
- Passion and experience in supporting older people, including those with dementia.
- Knowledge of the needs of older individuals, including people with dementia and their carers.
- Good IT, literacy, and numeracy skills.
- Understanding of safeguarding.
- Experience in the Care sector and/or voluntary sector.
- Ability to drive a minibus.
