To regularly assess, plan and organise delivery of care to ensure the safe and effective running of the community team, providing holistic care to people who have life limiting conditions and /or who are terminally ill and dying. Enhancing independence, quality of life and the support provision of a variety of therapies, offering practical and emotional support to all patients and their carers, whilst ensuring the highest possible standards of specialist and supportive palliative nursing care is delivered, responding to changing circumstances and needs.
- To manage the caseload of patients referred for fast track funded care with palliative and end of life care needs, providing holistic palliative nursing care as appropriate in conjunction with other specialist palliative care colleagues and the wider MDT.
- To undertake holistic assessments of patients, coordinate, and plan care to meet patient outcomes and, following any assessments, provide holistic care to patients, their families, and carers, liaising and referring on as necessary to other appropriate health or social care partners.
- To use advanced communication skills in delivering highly emotive information clearly, sensitively, and unambiguously with staff, patients, families and professionals by face-to-face, telephone and/ or electronic means.
- To provide clinical navigation and coordination as part of daily role.
- To attend and participate in caseload review/multi-disciplinary palliative care meetings around patient care management, both internally and externally.
- To support DNACPR discussions and decisions pertaining to resuscitation with patients and families

- To promote the use of the end-of-life tools/framework as agreed for use locally e.g., individualised Care Plan, PPD, Advance Care Plan, Gold Standards Framework.
- To ensure the safe custody and administration of drugs in accordance with Havens Hospice procedures and the law
- To undertake a variety of other clinical procedures such as dressings, enteral feeding, venepuncture, as required to ensure the patient needs are met (in line with competency framework)
- To back-up clinical decision making with theoretical and professional knowledge and communicate this effectively.
- To maintain professional competence through adherence to a lifelong learning philosophy and promote a culture of learning and professional development within the team.
- To develop and maintain palliative care competencies and frailty principles.
- To ensure maintenance of professional revalidation.
- To take part in clinical audit and the development of clinical guidelines.
- To report compliments and complaints as per Hospice policy.
- To participate in ensuring staffing rotas are adequately manned and good working relationship exists with locum agencies.
- To rotate into the other adult hospice teams as required and assist with other services to ensure continuity of service to patients.
