This is a fixed term vacancy to cover maternity leave between 01/04/24 and 01/09/24, either 5 months 22.5hrs per week, or 3 months 37.5hrs per week.

You would be joining a well respected and well established team of highly knowledgeable practitioners on a full time fixed-term contract or secondment (to cover maternity leave up to September 2024), working to support those who attend hospital in crisis. The role involves thorough assessment of mental state and of risk, education and challenging stigma, working towards a smoother journey for the person accessing services, and establishing appropriate follow on support.

You’ll be joining the MHLT based at St Richard's Hospital; providing care under the mental health crisis pathway to meet the needs and preferences for people accessing urgent care. You will have a primary base with the Liaison team covering 24/7 care.

You should be prepared to work shifts which can include long days or nights.

Liaison work with patients in mental health crisis and commonly see people when they are experiencing or have experienced: self-harm, suicidal ideation, the consequences of alcohol and drug use, including when co-occurring with a mental health problem, dementia or delirium, a severe mental illness, or social vulnerability that may have a mental health problem or trauma as a component or root cause, for example, homelessness or domestic abuse.

Identify, assess and respond to mental health crises in line with national standards that includes working with hospital staff to ensure the patient is safe and supported while waiting for and during an assessment culminating in a collaborative care plan.

Treat the symptoms of the mental health crisis in line with NICE recommendations

Provide access to ongoing support by offering follow-up care with services that meet needs.

Provide service-level support by establishing links with other emergency health and social care services, including those provided by the voluntary sector

Provide training to other healthcare professionals who may need to respond to mental health crises

Enable data, record and information sharing to ensure rapid, appropriate and safe treatment and follow up and preferences if known.

This is an exciting role that welcomes registered mental health nurses, occupational therapists or social workers with relevant professional registration who possess strong mental health assessment and therapeutic skills. Previous experience of working in acute/crisis mental health services is desirable although not essential.

Respond to referrals from A & E and the General Hospital wards, carrying out comprehensive biopsychosocial assessments including risk assessment. To make appropriate clinical decisions based on need.

To prioritise work according to urgency of referral and to be able to work both in a team and unsupervised.

Signpost appropriately to other services and make appropriate onward referrals.

Working closely with relatives and non-mental health colleagues in the general hospital and a wide range of community services in order to facilitate a safe and effective plan of care.

All UCPs will have an active role in terms of reducing stigma and challenging the culture and climate for those who have mental health problems, promoting a positive view of mental health and the parity of esteem agenda.

To provide effective clinical assessment and intervention that is responsive to service priorities and service user (and carer) needs, minimising the need for admission to a mental health unit, and/or prolonged occupation of general hospital beds.

Alongside other team practitioners, provide expert advice and support to the staff of the acute hospitals trust, ensuring that service user and carer needs are met.

To be responsible for organising and prioritising own workload related to both referrals into the service and the existing caseload, ensuring service users are offered the most appropriate skills and resources available to the team at any given time.

To ensure that all service users on the caseload have an appropriate care plan and risk management plan, which has taken account of service user and carer views, ensuring the regular review of both.

To ensure high quality communication and effective relationships within the Urgent Care team, as well as with other mental health services, general hospital colleagues, carers and all other agencies addressed within the role.

To work in collaboration with general hospital staff in facilitating effective and timely assessments and interventions for patients who might otherwise require admission to a mental health inpatient unit, or whose discharge from the acute hospital might be delayed whilst awaiting mental health assessment and allocation of appropriate mental health services.
