This exciting new post serves the Health Board, Local Authority and third sector partners as part of a small team of transformation professionals, dedicated to driving the integration of health and social care services in Pembrokeshire.

You will work closely with operational managers to assure the highest quality delivery of innovative transformation projects designed to make a difference to the health and wellbeing outcomes of our population.

Your main duty will be quality assurance. You will help clarify objectives and identify outcomes, support your clients to break down and sequence the phases and activities most likely to deliver these objectives and manage the measurement and continuous review process to ensure everything remains on track.

**You are likely to be a great join-upper of dots**: able to envision how activities connect and overlap, break down processes and facilitate problem solving. You will anticipate issues and mobilise your client’s team and stakeholders to mitigate or resolve them, being willing to lead the way and take responsibility yourself.

While the greater part of the job is ensuring that change happens, there is a rewarding analytical and reporting element that you will design to help your clients determine the success of their project. Nothing signals more how much this job matters than the fact that you will be at the centre not only of implementing the change but also helping your client’s team decide how successful the change has been and whether resources should be diverted to a more promising prospect.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Our 11,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales. We do this in partnership with our three local authorities and public, private and third sector colleagues, including our volunteers, through:
**Four main hospitals**: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest;
**Seven community hospitals**: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;
48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;
Numerous locations providing mental health and learning disabilities services.

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
