You will be joining a proactive skill mixed team working together to deliver the Healthy Child Programme. As part of the 0-19 Service which with other local statutory, health and voluntary agencies to provide includes Health Visiting and School nursing you will be working closely accessible, person centred packages of care in response to individual and local need. You will be working in partnership with Wirral Local Authority to take the local children’s agenda forward.

Works across Wirral supporting delivery of the Healthy Child Programme 0-19.

Manages a delegated active caseload, working as a member of a skill mixed team, responsible for interacting constructively with team members and organising, prioritising and planning their work. This includes, arranging visits and supporting delivery of universal services and additional packages of care. Facilitation and delivery of clinics and group sessions as required, including evenings based on service need.

Undertakes Healthy Child reviews/contacts and promotes uptake of immunisation as per Trust and professional guidelines.

Supporting delivery of identified packages of care, providing support and advice to clients ensuring family Health Visitors/School Nurse is informed of progress and outcomes.

Provides a holistic approach to promoting health and wellbeing whilst undertaking unsupervised contact, recognising when to liaise and report to Health Visitors/School Nurses and Team Leader

Provides advice and on-going support to parents to enable them to make informed choices in areas such as: diet, parenting skills, sleep, behaviour and safety.

Identifies individual client needs and provides advice to empower parents/carers by considering a range of options to minimise risk-taking behaviours for example: alcohol misuse, smoking, obesity and sexual health.

Assist in the development of materials and activities for inclusion in health promotion and behaviour change programmes. Prepare health education displays and maintains material/leaflets.

Deliver a range of health promoting programmes and activities for inclusion in a variety of settings including: mental wellbeing, physical activity, stop smoking support, oral health, food and nutrition and lifestyle/weight management.

Contributes to the completion of the family health review at identified contacts.

Refer clients to other services for more specialist support as appropriate.

Responsible for organising, planning and implementing the Child Health Clinics and drop in clinics in various settings and follows the Trust’s Infection Control and Health and Safety policies.

Promotes positive health to individuals, families, groups, communities and relevant agencies.

Plan organise and attend a range of activities and participate in presentation of health promotion activities to children in schools, the general public and other agencies

Responsible for the safe use of equipment, including transportation, storage and reporting faults or concerns to the Team Leaders or Health Promotion Specialist.

Works in accordance with current Trust policies and procedures, ensuring safeguarding procedures are maintained.

Assists in the learning experience of students in their community placement and participates in the orientation of new team members.

Liaises with other statutory agencies and voluntary agencies as appropriate.

Attends regular management/caseload supervision with line manager.

Participates in the annual appraisal and PDR as per Trust policy.

As a lone worker adheres to the Trust health and safety and lone working policies.

Attends regular management/caseload supervision with line manager.

Possesses the Trusts core competencies for role and attends mandatory training relevant to role

Maintains accurate paper and SystmOne computer records as outlined by the NMC guidelines.

Works in accordance with the Trust’s policies/procedures and clinical guidelines.
