You will speak with people who use our services, and their families/important people to them via telephone, responding when someone's usual routine has changed or in the event of an emergency. You will also visit people in their own home where required.

You will become a part of a friendly, supportive and caring team of Wellbeing Assistants dedicated to improving the wellbeing and quality of life of people living in rural Rushcliffe (And surrounding villages). Where many people are at risk of feeling isolated and often, do not have access to traditional domiciliary care services.

About you

- You can easily travel around Rushcliffe and surrounding villages when needed (this is role is likely to be a minimum of 50% office based).

**Key Duties**
- You will work as part of a self-managing team - so it’s important you can use your initiative and problem solve. In turn you will be trusted to be autonomous and do great work.
- You will monitor data transferred from our Tec2Care sensors that are installed in people’s homes. Making timely, appropriate decisions regarding any action that needs to be taken depending on activity levels and the environmental conditions of the home
- You will make regular check in and wellbeing telephone calls to people where this is part of their package
- You will visit people in their own home to support with wellbeing where this is part of the package (visits could include support with personal care).
- You will make emergency responses visits to people's homes as required
- You will work flexibly with the people we support to deliver visits and services at times that suit them. Managing and changing your workload to meet their individual needs.
- You will know of/learn about local statutory, voluntary and paid for services and businesses within Rushcliffe to add our directory of local trusted services for us to signpost people to
- You will liaise with these servicesto enable people to access timely and appropriate support/advice.
- You will promote the people who use our services as partners and experts in the services we deliver
- You will record and report using systems to evidence the work we do for individuals and the community and identify any areas for improvement.
