**We have an exciting opportunity at the Derbyshire Recovery Partnership for someone who is passionate about helping to make a difference to the lives of individuals using drugs and/or alcohol. This role will be based at one of our bases in Derbyshire; High Peak, Chesterfield, Ripley, Ilkeston or Swadlincote, **working 37.5 hours per week, 9am-5pm Monday-Friday, on a permanent contract.**

We are looking for a Prison Continuity of Care Worker to manage a caseload of service users involved in the criminal justice system and presenting with drug and alcohol issues. The role will involve multi-agency working which will underpin your prison in-reach, community engagement and outreach work. A vital part of this role is to sufficiently engage service users in prison, develop meaningful person centred release plans and ensure seamless continuity of care from prison to community treatment.

The aim of these roles is to contribute to overall community safety from drug and alcohol related crime. You will deliver 121 care-planned support and facilitate group work sessions, using appropriate models, interventions and approaches tailored to meet the individual needs of individuals on your caseload. Furthermore, you will support them along their individual, person-centred journey to build recovery capital by partnership working with other support agencies and local projects.

We are keen to hear from you if you have criminal justice/substance misuse experience as well as anyone with transferrable skills who is motivated and passionate about recovery.

Office base for the purpose of expenses can be agreed at interview and staff care parks are available at all bases.
