Key Responsibilities:

- Provide support in accordance with individualized support plans, respecting cultural preferences and personal wishes.
- Assist individuals in managing their homes, including tenancy maintenance, and facilitating smooth transitions.
- Offer compassionate assistance with personal care tasks, adhering to support plans and individual comfort levels.
- Empower individuals to make informed choices and life decisions, while upholding the principles of the Mental Capacity Act.
- Encourage participation in diverse home, social, leisure, and work activities based on personal choices.
- Aid in addressing health needs, such as medication management and medical appointments.
- Maintain accurate and up-to-date records, ensuring meticulous documentation.
- Offer guidance on financial matters as needed, following Hft guidelines.
- Champion the health, safety, and overall well-being of the individuals we support.
- Foster positive collaboration with colleagues, families, external organizations, and professionals.
- Embrace innovative approaches, including personalized technology, to enhance independence.
- Always uphold strict confidentiality standards.
