The post holder will work closely with Borough Based Partnership colleagues; Public Health, Clinical Directors and leads, Primary Care, Local Authority and will align work with the Health & Well-being Strategy for Ealing published in 2023 and the West London Trust Strategy. There will be a particular focus on reducing inequalities, improving outcomes and developing an evidence base of service pathways, including working in partnership with LA and Community Voluntary Sector.

The Post-holder will, with colleagues, lead on improving the health and well-being of the residents of Ealing. Annual objectives will be set with Director of Integrated Community Health Services and Director of Public Health.

Be responsible for improving or changing services or environments to improve health from the basis of assessment of need, understanding of methods of meeting those needs and approaches to getting ownership across organisations and communities. Within the Borough work will focus on aligning to Health & Wellbeing Strategy and WL NHS Trust priorities, including inequalities.

Be a member of relevant Ealing partnership groups, e.g. JSNA steering group. There will be a particular focus on reducing inequalities, improving outcomes and developing an evidence base of service pathways, including working in partnership with LA and Community Voluntary Sector.

Act as a change agent to enable delivery of relevant outcome indicators from public health, NHS and social care outcome frameworks.

Be expected to work across organisations, be able to influence budgets held by those organisations and effectively advocate for change, to improve population and planetary health
