CACT is the commissioned provider of a Social Prescribing service for Greenwich, focused on providing support for patients with non-medical needs, often complex, who are frequent visitors to primary care. The service is for adults over the age of 18 whose needs meet referral criteria and is being delivered by CACT but in close partnership with a range of other providers.

Social prescribing enables all primary care staff and local agencies to refer people to a Live Well Coaches. Live Well Coaches work as part of PCN team and give people time and focus on what matters to the person as identified through shared decision making or personalised care and support planning. They connect people to statutory and community groups and agencies for practical and emotional support.

The Live Well Coaches work within multidisciplinary teams and collaborate with local partners to support community groups to be accessible and sustainable and help people to start new groups. Live Well Coaches typically support people on average over 6-12 contacts (including phone calls, meetings and home visits) with a typical caseload of 200-250 people per year, depending on the complexity of people’s needs. CACT manages a mature social prescribing scheme with complex cases manged though a multi-disciplinary team.

Patients over the age of 18 will be eligible for support. The post holders are expected to have excellent communication skills, a strong understanding of key issues affecting health and wellbeing, and experience that lends itself to working on a one-to-one basis in a health and care context.

Please note that this role is Monday to Friday but Evening Shift work from 2pm - 10pm. The role is based at the Urgent Treatment Centre, Queen Elizabeth Hospital in Woolwich.
