An opportunity has arisen for a Registered Nurse/ Allied Health Professional to join Lincolns Discharge HUB in its mission, to support the wards in delivering timely and effective discharge. If you are an experienced Band 5 looking for their next challenge or a Band 6 interested in trying something new, this may be the role for you.

Working within the Integrated Discharge HUB you will be part of a team working to provide expert advice on Complex Discharges to the Trust and providing support to patients, relatives and staff through admission to discharge. Within this team you will have the opportunity to develop working relationships with multiple system partners (Adult Social Care, CCG, Continuing Health Care, Local Councils and others)

This is a role for anyone who has worked in a ward area or a community setting and now wishes to develop their skills further, by specialising in the Complex Discharge field. An understanding of the discharge process, would be advantageous. You will be required to be an expert communicator and passionate about improving the experience of our patients, their families and our Team members.

The post holder will have a unique role in forging further links between these partner organisations and will be experienced at building cross sector relationships.

This post is 37.5hrs a week involving rotational shifts covering a 7-day service (including Bank Holidays and Weekends).
