An exciting opportunity has arisen in the Cancer referrals office for a motivated, enthusiastic and self driven individual to undertake the role of 2ww Office Team leader and Multidisciplinary team (MDT) Co-Ordinator support officer

We are seeking a motivated and enthusiastic person to manage the 2ww team, ensuring the booking of all GP referrals in line with the cancer waiting time guidance and supporting MDT Co-Ordinator's to ensure that all suspected and diagnosed cancer patients that are to be treated by outside providers are transferred in a timely fashion.

There is particular focus on managing patients to ensure they are transferred and the package is received by the treating trust in line with the national target implemented with particular reference to the national minimum dataset for cancer

You will be part of the team in the Cancer data & referrals office and be expected to collect accurate clinical and waiting times data within the deadlines. You are also expected to support with tracking the pathway of the suspected cancer patients and covering the Cancer weekly meeting to feedback on the progress.
- MDM outcomes are appropriately recorded on EPR and on Somerset Cancer Register (cancer patient database used at the Trust) immediately at MDM (live recording) or immediately after MDM.
- The relevant imaging is reported and available on the PACS system and available for the meeting.
- All required medical notes and imaging are available for the meeting.

Patient Pathway Support
- Ensure referral packages needing to be sent to a partner Trust are sent within one working day following MDT in accordance with minimum dataset as described in local ITT Policy
- As directed by MDT coordinators chase feedback from the tertiary centre

regarding patient’s treatment plans including dates.
- To ensure patient confidentiality is maintained at all times
- To work with the MDT team to establish a computerised route to collect all data, minimising paperwork required for all data collection
- To act as a patient navigator, and follow the patient’s journey from diagnosis to treatment
- To support the smooth transition of diagnosed patients to a tertiary treating Trust and ensuring that all InterTrustTransfer minimum datasets are complete
- To identify potential breaches and escalate to the relevant member of the tea
