**Living Options Devon - Job Description Job Title**: Patient Support Officer Waiting Well Project Responsible to: Operations Co-ordinator (Waiting Well Project) Hours: Full time (37 hours per week) part time hours considered Salary: £25,991 Full time equivalent Job location: Exeter office and remote home working Contract term: Fixed term contract to 31/3/24 likelihood to extend funding permitting Period of Notice: One month Living Options Devon Background/context: Living Options Devon (LOD) is a charity and a company limited by guarantee founded in 1990. We exist to ensure people with physical and/or sensory disabilities and Deaf people with sign language can make an active and equal contribution in society. Specifically, we: Encourage people to feel more informed, valued and confident to take part in society through relevant training and support; Enable people to identify priorities and develop user-led services; Engage people in effective communication with local/national service commissioners and providers; and Empower people to raise awareness of what society needs to do to provide equality of opportunity both locally and nationally.

Aiming to improve their lives through community support and connectedness whilst they wait for NHS treatment, diagnostics or for a medical appointment. Key Tasks internal: To provide up to date information and/or advice for people referred to us, researching as necessary To log all client contacts onto the LOD Customer Relationship Management (CRM) system in the agreed manner To provide details of Service user contact for evaluation purposes as requested To follow the key processes required for effective referral management To manage own case load To work predominantly independently and be proactive to engage with other team members for mutual support Other Duties Attend and participate in regular staff and team meetings. Attend appropriate training courses where identified. Attend and participate in regular work appraisals and any other support systems as appropriate.
