Taurus Healthcare are looking to recruit a Social Prescriber to be part of our ‘Talk Wellbeing’ outreach service to help to tackle neighbourhood inequalities and improve access to our underserved communities. The service aims to increase early identification of health issues through NHS Health Checks, covid vaccinations and other prevention interventions, as well as supporting residents to register with a GP practice and wider signposting to other organisations and community groups.

Social prescribing empowers people to take control of their health and wellbeing through a strengths based personalised care approach. to the Link Worker gives time to people to focus on ‘what matters to the patient’ and takes a holistic approach to an individual’s health and wellbeing, connecting people to community groups and statutory services for practical and emotional support.

Multi-disciplinary teams will provide personalised support to individuals, their families and carers to take control of their wellbeing, live independently and improve their health outcomes. Outreach work helps us to get to the heart of our underserved communities to improve outcomes for individuals health and wellbeing.

The Social Prescriber will offer practical tools and techniques, advice, knowledge and encouragement to enable and motivate people to change behaviours and achieve better health and lifestyles choices and achieve goals for themselves, their families and the wider community. The Social Prescriber will also play a key role working with communities to build relationships and support identification of areas for outreach work.

The Social Prescriber will also be expected to develop skills in taking and recording physical observations to include blood pressure, heart rate and rhythm, weight, and finger prick analysis of cholesterol levels within the remit of an NHS Health Check, for which there will be additional training and support, including competencies for conducting an NHS Health Check and a Making Every Contact Count approach.

**The Social Prescribing Link worker will:

- **
- take a holistic approach, based on the person’s priorities, and the wider determinants of health.
- co-produce a personalised care and support plan to improve health and wellbeing.
- introduce or reconnect people to community groups and services, both over the phone and in person.
- draw on and increase the strengths and capacities of local communities, enabling local VCSE organisations and community groups to receive social prescribing referrals.
- Work with underserved communities to understand barriers to accessing statutory services
- Draw together reports using Word and Excel for Herefordshire General Practice demonstrate how social prescribing and the Talk Wellbeing service can improve outcomes for patients.
- Work with community champions to identify opportunities, locations and groups for the Talk Wellbeing service to engage with.

**Main aims of post**:

- Help to identify people in need of further health intervention in line with the Talk Wellbeing Programme.
- To promote the Talk Wellbeing Service, its role in addressing inequalities and reaching underserved communities, including identification of undiagnosed conditions through Health Checks and increasing patient registrations in General Practice self-management and the wider determinants of health to members of the PCN and other agencies.
- Build relationships with PCNs, Community Action Networks and the Community Partnership to identify opportunities to collaborate and address inequalities through an outreach approach with the Talk Community Service.
- Be proactive in developing strong links with all local agencies and VCSE organisations.
- Work in partnership with all local agencies to raise awareness of social prescribing, Talk Wellbeing and how partnership working can reduce pressure on statutory services, improve health outcomes and enable a holistic approach to care.

**Hours**:37.5per week.

**Salary**:£27,583.00 - £30,766.00 per annum.
