We are looking for a Band 4 Advance Practitioner to join our Community Rehabilitation Team in Ashford.

The Community Rehabilitation Teams across Kent are multi-disciplinary teams that include Occupational Therapists, Physiotherapists, Speech and Language Therapists, Specialist Nurses (within East Kent) and Therapy Assistants.

We work within patients’ homes, rehabilitation groups, Community Hospitals and other care settings, to ensure effective discharge planning and a seamless rehabilitation pathway. We provide holistic assessment of patients’ needs and help patients achieve maximum independence in daily living activities and improved well-being

In your role you will complete holistic assessments for patients with Long-term and/or Acute medical conditions, and support delivery of Physiotherapy, Occupational Therapy and Speech and Language Therapy (SLT) programmes under direction of the registered healthcare professional.

You will implement and evaluate therapy interventions, supporting the delivery of the Community Rehab Team.

We provide short-term rehabilitation to enable patients, where possible, to remain in the community rather than be admitted to hospital or long-term care, and to live as independently as possible. We have close links with Acute Hospitals to ensure patients are able to continue their rehabilitation in the community following discharge home
