Discharge Facilitator

This is a patient-facing role with the majority of your time spent in ward areas liaising with patients and their families & members of the ward team. Whilst you won’t be involved in delivering patient care, you will see every day the impact you make and we know how proud you will be of the role you undertake.

It doesn’t matter if you haven’t worked in a hospital setting before - we can teach you everything you need to know. This role will really provide a good introduction to expand your knowledge and understanding of how our hospitals operate.
You’ll ensure discharge planning commences from day one of admission liaising with the multi-disciplinary teams and family to ensure patients spend the minimum amount of time required in hospital.
Your effective communication skills throughout all of this will be key. You will be the main point of contact relating to the discharge of patients on your ward area, linking in with social care and therapy services, utilising our IT systems. You’ll need to use your initiative of course, but you will also have a supportive team around you to help. Your strong organisational skills will also come into play as you manage your workload and ensure tasks are completed timely and any concerns escalated.
Our services are 7 days a week - teams are deployed to work on flexible shifts at either of the acute sites depending on business need.

Ability to work as part of a team liaising with inter-disciplinary team members regarding the planning of discharge for relevant patients.
Ensure completion and delivery of relevant documentation to assist with the discharge process.
Communication with patients, relatives and other staff members concerning issues related to discharge planning. Ability to deal with communication issues.
Liaise with other agencies/patient/relatives in order to provide effective communications to assist with timely discharge of patients.
Completion of relevant patient documentation in order for an effective discharge process.
Communication with a range of clinical and non-clinical staff concerning the discharge process for patients.
Ensure that the social circumstances / discharge arrangements of each patient have been assessed and documented within 24 hours of patient being admitted to the ward environment.
Monitor the progress of referrals/assessments that are required to assist with the discharge process.
Follow up any outstanding requests that are causing a delay with the discharge process for the patient.
Ability to analyse and interpret information, pre-empt and evaluate issues and recommend an appropriate course of action, seeking advice from senior colleagues if required.
Support ward teams by facilitating the process of requesting TTO’s / Transport/ Discharge Summary to assist with a timely discharge and avoidance of any delays.
Ensure that all relevant paperwork is completed by the appropriate persons to ensure an efficient and timely discharge.
Support chasing results for tests where this could delay achieving discharge.
Liaise with other care establishments to expedite any referrals or assessments that have been requested including the repatriation of patients.
