More detail about the role Tiverton Hospital has two Intermediate Care wards specialising in the rehabilitation of our Community residents following an acute Hospital admission. Our wards use an integrated team approach with our Nursing colleagues to support the patient to achieve their goals and maximise independence. The Onward Care Clinician takes lead in admissions and discharges to and from Tiverton Community Hospital.
We have a wide and varied caseload, working with patients with long term, Neurological and Orthopaedic conditions, as well as dementia, and advising on manual handling methods to support their rehabilitation needs and their transition home. We encourage integrated working with our NHS colleagues in Multidisciplinary teams and also our DCC partners in the Social Care sector, third sector to deliver a high quality service in our locality. The post holder will be responsible for the supervision and development of Band 5 Nurses/Therapist and unregistered Nursing and Therapy Support Workers. We are committed to the continuing professional development of all our team members and offer a supportive environment with regular supervision and encourage on-going external learning as well as providing internal CPD groups, training and peer support.
