At Thrive Tribe we deliver award-winning healthy lifestyle and wellbeing programmes that make people happier and healthier We have an exciting opportunity for an **Adult Weight Management Practitioner **to join our One You Lincolnshire service. One You Lincolnshire is an Integrated Healthy Lifestyle Service which has been supporting local residents, since July 2019, with Smoking Cessation, Weight Management, Alcohol Reduction, Physical Activity and Health Coaching. More recently the service has expanded to offer Child and Family Weight Management, and Strength and Balance for adults aged 65+.

A typical day as an Adult Weight Management Practitioner might include:

- Delivering a planned evidence-based intervention to obese clients, that focuses on nutrition, movement, mind, sleep and alcohol, with the aim of helping the clients to adopt healthier habits and lose weight.
- Complete 1-to-1 phone calls with clients to get them started on their gloji group journey, assessing their motivation, confidence and commitment to the programme.
- Accurately recording and monitoring data in a timely manner using basic IT skills.
- Providing 1-to-1 telephone support for clients where identified as appropriate.
- Collaborating effectively with One You Lincolnshire team members, to ensure clients are supported as they move between the different elements of the Integrated Health Lifestyle Service.
- Ensuring due consideration is given to Information Governance and Clinical Governance legislation, requirements and guidance.
- Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values

To work with us in this role **you will need to have experience of supporting individuals to make healthy lifestyle choices through nutrition, physical activity or health coaching.** Nutrition qualification would be advantageous. **Contract**: 1 x 0.2 Full Time Equivalent (FTE) permanent (7.5 hours per week/1 day per week) or 1 x zero hours contract

**Salary**:
0.2FTE : Starting at £22,500 per year (rises to £24,000) pro-rata

Bank staff: Admin tasks are paid at £11.50 per hour and group delivery is paid at £18 per hour

**Location**: This job is based Lincolnshire, with travel across the County (car owner required).

**Working Pattern**: This role will require some flexibility around evening work (no later than 19:45
