Efficient patient flow is essential for all hospitals to ensure that patients are being cared for in the right place at the right time. This is an exciting and challenging role for a dynamic individual who can work autonomously and as part of a team across various sites within SBUHB.

SBUHB operates a D2RA model and whenever possible the patient should be assisted to return to their usual place of residence. This may require the short or long term assistance of various agencies/services that operate within the community. The role of the TOCALS practitioner is to co-ordinate the discharge direct from the acute setting by liaising with the MDT, patient and their relatives in the first instance.

The role also requires the individual to assess/screen patients for transfer to NPTH wards that incorporate rehabilitation, stroke and medical wards.. Additionally, the screening of patients that require repatriation to NPT from national and international hospitals is also required.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Assess patients at acute hospital sites with a view to supporting discharge
Provide advice and liaison between community services and acute hospital sites to facilitate a safe discharge.
Communicate with patients and their families about discharge planning
Where discharge not appropriate to assess patients with regards to their suitability to transfer to Neath Port Talbot Hospital.
Supervise junior staff and be able to teach qualified and unqualified staff, including basic and/or post registration students.
Develop professional/working relationships with colleagues and other members of the multi-disciplinary team.
