Community Assistant Practitioner

An exciting opportunity has arisen within the Virtual Ward Team to work as a Community Assistant Practitioner within a multi-disciplinary and multi-agency team.
We require an enthusiastic and motivated assistant practitioner to join our established friendly team. The role will require a reliable and flexible individual who is able to work independently and support others to ensure the delivery of a quality service.
To assist in the delivery of high standards of health care to patients within their own places of residence, by following prescribed care pathways over a 12 hour period.
You will be fully supported by the wider Virtual Ward Team with all aspects of the planning and delivering of care for this complex group of patients.
This is a 7 day a week service and some weekend working will be expected. Hours of service delivery, 8am-8pm
Norfolk Community Health and Care embraces the principles of Improving Working Lives and all staff will be required to adhere to the standards laid down, in this initiative.
Maintain appropriate patient and clinical records in line with the Trusts policies and procedures, and in line with the agreed service specification. In line with the annual personal development plan the post holder will be expected to undertake any training or development required to fulfil their role.
Ensure that all patients, clients and colleagues are treated at all times in an equitable manner, respecting diversity and showing an understanding of diversity in the workplace.
The post holder must carry out their responsibilities with due regard to the Trust's Equality and Diversity Policies, Procedures and Schemes and must ensure that equality is promoted at all times.
**As part of Virtual Ward you will be required to**:
Engage with mobile working
Become expert at SystmOne and other technological systems i.e Feebris.
Engage with referral to discharge standard processes
To be aware of the demand and capacity model which will reflect workload needs at any given time to promote increased capacity.
Engage in admission avoidance and supporting Virtual Ward discharge processes from the acute sector when required.
Develop and support the role out of training in all points of remote testing across all N+ W Localites. Including the sign off of competentcies.
Key Areas of Responsibility
To contribute to the delivery of care pathways for patients with long term conditions, palliative care and rehabilitation needs for example, to achieve quality of life and independence where possible.
To work within the integrated team to support early discharge from hospital
To work within the integrated team to prevent unnecessary admission to hospital
To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
Main Responsibilities
Assess patients with straightforward requirements based on predetermined department protocols and what remote technological equipment would be required.
Support staff to Prepare for, carry out and monitor assessments and treatments in specified clinical areas, and discharge in line with predetermined department protocols.
Modify and progress intervention / treatment using own clinical reasoning, notifying a qualified practitioner accordingly.
Monitor patients and promptly alert a qualified practitioner when there are unexpected changes.
Demonstrate problem solving, and contribute to the solution, working with colleagues.
Make basic judgements on patients’ response to prescribed treatment, and report findings to a qualified practitioner.
Plan and prioritise own visits to delegated patients, and ensure these are documented on SystmOne.
Ensure that SystmOne inputting is completed on a daily basis.
Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in order to gain understanding, consent and concordance.
Be required to use tact and persuasive skills in order to gain the patient’s co-operation in their treatment/care/management plan.
Liaise with the wider Virtual Ward MDT and regularly communicate with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector as required to ensure identified needs are met and care co-ordinated appropriately.
To participate in multi-disciplinary/multi-agency meetings as appropriate, e.g. Gold Standard Framework.
Communicate with patients/carers by exchanging factual information, reassurance, tact and empathy.
Establish and maintain appropriate working relationships with colleagues, patients, carers and other health care professionals.
Demonstrate dexterity and coordination when undertaking treatment of patients where accuracy is important, e.g. taking blood sugars, venepuncture, supervising transfers, passive exercises.
