Service Co-ordinator

**PURPOSE OF THE JOB**
The purpose of this job role is to provide support to adults who have mental health issues (and where appropriate their families/children) across the City for up to 12 months. The level of support will be medium to high support for people with multiple and complex needs (including dual diagnosis) aged 18 and over.
The service will provide assertive prevention support to service users living in their own homes, in all tenures, to develop their resilience and remain independent in the community.
The post holder is responsible for developing and delivering a high-quality person-centred support service, with jointly agreed outcomes and will be responsible for managing their own caseload.
You will also be expected to contribute to the maintenance and development of the Association’s values, culture, and ethos.
**We have two positions available one for 37.5 hours per week and one for 22.5 hours per week.**

**KEY RESPONSIBILITIES**
**Provision of Support**
- To help process referrals from a broad range of agencies but priority must be given to Sheffield Health and Social Care Trust, in particular SPA, Recovery Services and IAPT.
- Assist Service Users to achieve jointly agreed objectives set within their support plan and adopt a multi-agency approach and liaise with other agencies as and when required.
- To support and assist Service Users in maintaining and sustaining their accommodation and working in partnership with other relevant agencies.
- To deliver a combination or 121, group work and/or drop-in sessions for the Service Users, helping to create and maintain HUB’s and surgeries to deliver these sessions and help build capacity to deliver service KPI’s.
- To manage and maintain a case load of Service Users as allocated and as determined by the needs of the service.
- To maintain individual records to a high standard and record outcomes on both Adullam and Commissioner systems.
- To assist Service Users in all practical matters to achieve their outcomes and by providing each Service User with an average of 4 hours per person per week.
- To advocate verbally and in writing on behalf of the Service User to ensure they receive the services and benefits to which they are entitled.
- To work closely with the relevant statutory and voluntary agencies to develop their resilience and remain independent in their community and sustaining their home. The service will take a proactive and assertive role in steering a journey to recovery and prevent or reduce dependence on paid support.
- To encourage Service Users in developing their skills for independent living including practical household tasks, interpersonal skills, budgeting and taking responsibility for paying bills.
- To enable and empower Service Users to undertake activities in the wider community and maximise opportunities in employment, training, education, and leisure. This in part will be achieved by developing your own networks in the wider community and promoting the service positively and proactively.
- To build capacity within the service through individual and group work but also by supporting the health and wellbeing agenda and the use of volunteering and co-production to supplement, enhance and improve services delivery.
- To review and monitor service delivery in conjunction with the line manager to ensure continual improvement, value for money and adopting a best practice approach.
- To promote Service User involvement, volunteering, and peer mentoring opportunities, aligned to a Co-production approach to motivate and enable service users to take up these opportunities.
- To maintain the highest level of confidentiality within the Association’s using relevant Policies and Procedures.
- Administration Duties
- To maintain accurate records in accordance with the Association’s Policy and Procedures and assist with the production of statistics, reports, and input into evaluation.
- To keep all Service User records up to date and in accordance with GDPR requirements, the Data Protection Act and the Association’s confidentially policy.
**As a member of the team**
- This service is delivered during office hours as a minimum, but the commissioner expects a flexible approach to meet the needs of all Service Users. So, there may be times when staff are expected to work some evening and/or weekend work.
