**KEY DUTIES AND RESPONSIBILITIES**: Client Work To accept referrals, assess need and make appropriate recommendations as part as a robust duty system. Provide harm reduction advice, information and support with an emphasis on safer drug use, safer sex, HIV, Hepatitis A,B and C and other blood borne infections Be responsible for maintaining a full case load of complex clients as the key worker. To make home visits consistent with need. To carry out comprehensive assessments, construct care packages with multi disciplinary colleagues where required, review care plans in a structured format and manage onward referral.

To assist and support medical and nurse prescribers during harm reduction and prescribing clinics. To provide a range of structured psycho-social interventions including CBT based brief solution focused therapy and motivational enhancement techniques. Make domiciliary visits to service users particularly for those with disabilities and to those with family responsibilities. Retain contact with service users whilst they are accommodated in residential rehabilitation units including visits when required.

To create and facilitate group work interventions. To provide a needle exchange service. Demonstrable commitment to service user involvement. To work with a range of professional in ensuring service users needs in terms of safeguarding and mental health are met.

Carryout needle exchanges, home visits and the running of clinics. Encourage and work with service users to move forward in their recovery Encourage and work intensively with service users to support them move away from maintenance treatment where appropriate Liaison To develop and sustain links with multi-disciplinary services including statutory and non statutory providers of Tier 2,3,4 drug services, criminal justice services and providers of wrap around services. To develop a network of contacts with relevant agencies to promote service accessibility for the client group. To attend relevant meeting to represent Inclusion and the team when required Administration To keep up to date records and information for the purpose of internal and external evaluation.

To provide information and statistics consistent with the Drug Action Team (DAT) and Inclusions requirements. To prepare and present reports when required to do so. Demonstrate an understanding of a patients physical health needs and be able to carry out blood pressure checks, infections control checks and ensure the service is in line with COSHH standards. Stock check/inventory re clinical items such as aprons gloves, drug testing kit, vaccines, oxygen bottles & masks etc (duty of care re H&S, infection control these items).

General To participate in regular supervision. To take part in annual appraisal and review based on the NHS knowledge and Skills Framework (KSF) To attend relevant internal and external meetings. To undertake any other duty relevant to the role as directed by the manager. To be prepared to work some evenings and weekends.

To demonstrate commitment to evidenced based practice and keep up to date with trends and changes in the drug field. To hold a full driving license and be prepared to travel to various locations consistent with service and client needs.

To take part in all relevant training provided by Inclusion, South Staffordshire Healthcare NHS Foundation Trust, the DAT and any other relevant agency. To contribute towards training others involved with the client group. To provide clinical supervision To provide mentoring support for new and/or inexperienced workers Line Manager provides supervision with objective for each worker to work individually on objectives, in line with Trust and occupational guidelines. Freedom to interpret how to achieve objectives.

Work from offices without direct management support. Continuous exposure to verbal and non-verbal aggression. De
- Escalation techniques, debrief sessions for incidents e.g. relating to overdoses occurring on site and off site.

Exposed to patients under the influence of drugs and alcohol abuse. SYSTEMS AND EQUIPMENT To contribute to the development of systems and interventions as and when required. To familiarise and use I.T. equipment as provided by Inclusion and to include ILLY (or any other introduced) and accurately record all required information.

To seek management advice where appropriate To identify training needs and utilise supervision effectively. To be aware of boundaries at all times.
