**What is Open Mental Health about?**

Open Mental Health is an alliance of local voluntary organisations and the NHS. We are working in partnership to ensure that residents of Somerset get the support they need, when they need it.

Our shared ambition is to ensure that people living with mental health problems get the right support at the right time. Working together, we support people to live a full life, by enabling access to specialist mental health services, housing support, debt and employment advice, volunteering opportunities, community activities and physical exercise, to help support and improve their wellbeing and quality of life.

**The Role**

**We are seeking an Intensive Community Navigator to work in our hospital discharge service - Next Steps. **Next Steps works with people to support timely and safe hospital discharges from inpatient mental health units. We are looking for people who have experience of working with people with long term mental health and complex needs and who have excellent communication skills.

The key elements of the role are:

- To support safe discharge by building strong and trusting relationships with clients.
- To in reach into hospitals and work alongside NHS and voluntary sector colleagues to complete and implement recovery plans.
- To navigate services around the client such a housing and debt support.
- To deliver trauma and psychologically informed support using person centred support plans and tools including: wellness recovery action plans.
- To maximise opportunities and access to accommodation, community networks, volunteering & employment.
- Connecting people to community resources, building resilience, improving wellbeing through targeted interventions, and coordinating and navigating social and clinical services around the individual.

**About you**:
You will need to:

- Have experience supporting people with complex needs, including mental health, within a community setting.
- Be a good communicator.
- Be great at building trust with those you work with
- Be keen to work as part of a multidisciplinary team to contribute to the delivery of this valuable service.
- Be able to work on your own initiative whilst being part of a larger dispersed team.
- Have strong organisational skills.
- Have a driving licence and access to a vehicle due to the geography of the area covered by the service.
