As a Referral and admissions Co-Ordinator, you will play a vital role in ensuring the smooth and efficient coordination of referrals to our supported living programs, contributing to the well-being and recovery of our Residents.

Be the main point of contact for all new referrals and inquiries and act as the lead for ensuring new opportunities are managed within the required timescales. Reporting on admissions and pipeline opportunities including what stage they are in the admissions process and ensuring appropriate funding/contractual mechanisms are in place prior to admission

**Key Responsibilities**:

- Administrate the sales database of commissioners, funders, referrers and their enquiries, ensuring the confidentiality and security of all Company information, paying particular attention to prospective client records under GDPR.
- Understand eligibility criteria, referral pathways and service provision, understanding service thresholds and responses to different level of need and effectively manage the referrals process into the service
- Work alongside the operational colleagues to share referral information and instigate process of assessment
- Maintain a live record of enquiries/referrals to show pipeline opportunities and at what stage they are in the admissions process
- Arrange pre-admission assessments on behalf of the clinical team and ensure assessment team has access to all relevant information prior to completing the assessment.
- Maintain regular communication with referrer to ensure they remain aware of how the referral is
progressing and ensure that all contact is logged on the CRM system.
- Send out completed assessment pack to referrer once handed over by assessment team and follow-up appropriately to ensure information is received and to field any clarification questions raised
- Assist the business development team in obtaining a signed contract and invoicing details for each confirmed client admission. When funding has been approved, liaise directly with funding authority to
establish billing information and what they require to setup payment of future invoices.
- Provide regular reports and analysis on enquiry/referral trends to enable the business to plan marketing/business development activity appropriate
- Implement systems and procedures to ensure the effective and efficient running of the admissions process

**The post holder is expected to**:

- Carry out other similar reasonable duties according to the business needs
- Undertake any training relevant to the requirements of the role
- Comply with health and safety procedures and safe working practices
- Adhere to data protection procedures, ensuring that sensitive business data remains confidential

**Working Hours**

The role is a full-time position working 37.5 hours per week. This role requires a level of flexibility
