The shift pattern is Monday-Friday 09.00-17.00 (22.5 hours).

(Please note the salary is Pro Rata) for part time hours.

The SCPT Nursing service operates an open access policy that is currently available Monday to Friday excluding Public Holidays. Care needs are prioritised and teams must be able to respond to both planned and unplanned interventions.

The post holder will participate in the care of service users/ families in their own home, or community setting under the direction of a SCPT Nursing team, taking into account the lifestyle, race, gender and cultural background, and factors which impact on physical, mental or emotional health. The post holder will represent the values and beliefs of the SCPT Nursing service as well as that of the organisation.

The Healthcare Assistant will be responsible for delivering safe, efficient and person centred care to service users/ families, as delegated and directed by the SCPT Nursing team. The post holder will support the SCPT Nursing team in the delivery of care to service users, families/ carers.

The post holder will promote a culture of engagement with service users, families/ carers, which includes as core principles and values an approach which is caring, compassionate, courteous, empathetic and respectful.

The post holder will promote the principles of self-care to clients, families and carers and empower them to access and use health resources in their community.

The post holder will:
1. Carry out clinical duties as delegated by the SCPT Nursing Team:

- Assist clients/ families in relevant settings to address health promotion activities that can positively improve health and wellbeing
- Assist with the delivery of a variety of clinics e.g. Immunisation, community drop-in
- Accurately report back to SCPT Nursing team following home visits or other client related work
- To signpost families to range of appropriate services to support their needs as identified in care plans
- To provide practical support for parents and children within the home or in terms of accessing other services and supports

2. Carry out other duties to support SCPT Nursing team:

- Assist with stock control and ordering of pharmacy and supplies as deemed appropriate by team
- Assist with basic administrative tasks as required such as filing and storage of records, compiling information packs, photocopying.

3. Attend staff meetings and clinical and mandatory training and be encouraged to record their training participation and learning experiences in Knowledge and Skills Framework

4. Participate in systems of practice support and personal development planning in line with the Knowledge and Skills Framework

5. Maintain accurate and up to date clinical records ensuring compliance with Data Protection Act and Freedom of Information Act in discussion with SCPT Nurse supervising case

6. Work within national, organisational and local polices, procedures and guidelines to ensure maintenance of safe, person centred and effective practices working practices for children, young people and families

7. To work with an awareness of cultural differences and be respectful of those with regard to colleagues, children, families, young people and communities

8. Establish and maintain professional relationships within and outside the SCPT Nursing Team

9. Refer to a senior member of the SCPT Nursing Team for any questions they may have relating to children, young people, families, or for any suggestions or complaints that they wish to raise.

10. Participate with other multidisciplinary/ multi agency staff who may require to gain an overview of the Health Support Worker role and induction of new staff into the team

11. Participate in audits required for organisational activity

12. As a member of the SCPT Nursing team the post holder will be able to support children and young people, families and carers in order to enable them to improve their health and wellbeing.
