**Job Summary**

The role also provides administration support to the PCN and practice teams, minuting meetings, managing calendar appointments, chasing up actions, running reports etc to ensure patients are accessing the services they need e.g. cancer screening. They will also support the weekly Care Home meetings, acting as the liaison between MDT members and Care Home staff.

**Primary Care Networks**

A Primary Care Network (PCN) enables the provision of proactive, accessible, co-ordinated and integrated primary and community care, improving outcomes for patients. They are formed around natural communities based on GP registered lists, serving populations of around 30,000 to 50,000.

The Torentum Primary Care Network was formed in June 2020. It includes 3 GP Practice’s, The Cleveleys Group Practice, The Crescent Surgery and The Thornton Practice, serving a population of approximately 33,500 registered patients in the Thornton Cleveleys area. The Torentum PCN team includes Care Coordinators, Social Prescribers, Pharmacists and Pharmacy Technicians, Mental Health Practitioners, Nurse Associates and an Advanced Clinical Practitioner who all work together as a supportive team to deliver high quality care to our patients.

**Primary Duties and Areas of Responsibility**

**Project Support**
- Set up and manage attendance at meetings; minute meetings and log actions.
- Prepare and present data from the patient records database (or other sources) to support decisions being made in projects.
- Run searches to identify and contact patients who need to attend for appointments or be referred to the other services.

**Direct patient facing work**
- Work collaboratively with staff across the PCN to identify and support patients’ attendance at appointments and wellbeing activities.
- Help people to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care.
- Receive and collate information maintaining accurate patient records.

**Care Home Multi-Disciplinary Teams**
- Administer the weekly Care Home meetings; managing the meeting agenda, taking minutes, re-registering new residents with the correct GP practice, and ensuring all information is circulated to team members in advance of the meeting.

- Receive and collate information from transfers of care (including hospital admissions and discharges), out of hours calls etc, for Care Home patients.
- Support the completion of new referrals by checking criteria, and where criteria have been met, referring to the MDT.

**Collaborative working relationships**
- Collaboratively work with other Care Coordintors, managing the workload as a team and covering duties for other Care Coordinator absences.
- Work as a cohesive team with the other staff in the Wellbeng Hub, receiving referrals and identifying which Wellbeing Hub roles could best support the patient.
- Actively work toward developing and maintaining effective working relationships both within and outside the PCN, communicating effectively with service users and their families/carers, building networks with GP practices, adult social care, hospitals, community pharmacists etc.

**Supporting Care Delivery**
- Follow up with service users and others involved to ensure all services and care arrangements are in place.
- Keep care records up-to-date as activities arise, uploading care plans to EMIS, identifying and updating missing or out-of-date information about a person’s circumstances etc.

**Confidentiality**
- In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately. The post-holder should always respect patient confidentiality and not divulge patient information unless sanctioned by the requirements of the role.

**Information Technology**
- Work closely with the Practice Managers and their teams to ensure that PCN documentation and other important documentation is maintained
- Work closely with the NHS Central Support Unit (CSU) and the Practice Managers to manage data collection, record appropriate coding, analyse reports etc to enable informed decision making and continuous quality improvement.
- Manage reporting requirements for the Care Home MDT.
- Collate information on referrals into the Wellbeing Hub to support the monthly reporting of PCN activities.
