We have an exciting new opportunity for a
**Polish speaking**
**complex**
**needs navigator**
***who will engage with a small cohort of service users who are rough sleeping and have complex.

We are an integrated drug and alcohol treatment service and provides access to a range of interventions and treatment options for adults experiencing problems with drugs and/or alcohol, promoting a focus on recovery and independence.

**Responsibilities**:
Working under the directive of the Team Leader.

**Responsibilities will include**:

- Engage with Polish Nationals who are rough sleeping or at risk of rough sleeping by participating in street-based outreach sessions at various times of the day and night.
- To co-ordinate and lead a pro-active solution focussed approach to clients identified as entrenched with a view to engage them in appropriate services and move them away from street-based living.
- To carry a small case load of complex clients and develop personalised action plans (identifying goals, aspirations and recovery focused interventions) that ensure these clients are discouraged from rough sleeping.
- Co-Facilitate Polish Speaking Groups for those who are at different stages of change.
- Assess suitability for referrals into services in terms of risk and specific needs.
- To work closely with all partner services, sharing appropriate information and ensuring that action plans are carried through.
- To maintain accurate and comprehensive service user records in line with Change Grow Live's procedures and protocols and the requirements of the local authority, maintaining confidentiality and sharing information with appropriate agencies.

**Salary**: points 28 to 30 (£30,316.74 to £32,412.09 plus £3,915.44 ILW (pro rata).

**Hours**: 37.5

**About you**:

- You will have some experience of engaging with individuals with multiple and/or complex needs and demonstrate a determined approach to finding solutions.
- Some knowledge of one or more of the following is required:
Homelessness

Mental Health

Criminal Justice System

Substance misuse
- To be able to communicate fluently and effectively with service users from Poland.
- Knowledge of social and cultural difficulties homeless Polish individuals face.
