As a band 7 Children's Continuing Care Lead (CHC) who is based within the Children’s Community Nursing service. You will oversee the delivery of child specific packages of care, either in the community, or school settings with the support of our new CHC continuing care coordinator.

As a band 7 you will undertake Children's Continuing Care assessments, liaising with Children, young people and families, colleagues, Health and Social Care professionals, colleagues in Education and any other team or services involved in the care of the child and young person.

If you are currently working as a Band 6 or Band 7 Children's Nurse with experience of working within the Children Community Nursing service and have strong leadership skills and knowledge to lead a team, we would love to hear from you.

**Main duties of the job**:
Managing the Children’s Continuing Care element within the Children’s Community at Salford. The post holder will deliver an effective and competent level of service that is consistently person
- centred focused for children and young people

Leading and developing the Continuing Care Team, working in conjunction with the wider Children Community Nursing Team (CCNT), external stakeholders, to implement effective service pathways, ensuring the delivery of key quality outcomes and strategic objectives.

Will line manage a skill mix team ensuring delivery of person centred assessments in order to identify any primary health care need which may result in eligibility for continuing care, ensuring the National Framework for NHS Continuing care for Children and Young People is implemented and complies accordingly to the principles and processes with onward referral to other agencies where appropriate.

**Detailed job description and main responsibilities**:
The role will ensure that the provision of care achieves individual outcomes and ensures the delivery of efficient quality packages of care that is aligned to the Personalisation and SEND agenda and supports the transition of young people into the appropriate adult services.
- The post holder will work in partnership with stakeholders and other professionals across both the public and private sector. This role is an integral part of fulfilling the requirements of the Children NSF for Continuing Care, which requires you to work with education, health and social care partners to ensure a team around the child ethos for continuing care is met. This would require liaison with the relevant health professionals, education, children with disability social work teams and other wider members of the team around the family (TAF).
- To act as ‘Lead Professional’ within the Continuing Care process, by liaising with other professionals to facilitate collection of written reports to inform the Continuing Care assessment process. This involves quality assure the completion of holistic and robust continuing care assessments
- An essential part of this role is to ensure each package of care through new or review assessments delivers the best possible care for the child, representing value for money within the agreed available funds. A strong business focus as well as clinical expertise is a key requirement for this role.
- The post holder will have responsibility for the management, and delivery of the Childrens Continuing Care Team for children and young people (CYP) who has been assessed as eligible for continuing care. This will be delivered in partnership with the CYP, their carer’s and other agencies involved.
