Gynaecology Oncology Patient Pathway Facilitator

A substantive position has arisen in the Maternity Division for a Gynaecology Oncology Patient Pathway Facilitator.

The post holder will be responsible for proactive management of the Gynaecology cancer pathways. Monitoring, reporting, and tracking cancer patients, ensuring the patients journey is progressed in a timely and efficient manner in line with National Cancer Waiting Times Targets.

Navigating patients will involve resolving delays in pathways at the earliest opportunity and when not possible escalating in accordance with the Gynaecology process.

The post holder will also attend MDT meetings, ensuring all relevant paperwork and clinical information is available and ensuring actions for the Gynaecology Clinical Teams are responded to in a timely manner.
- Navigating patients through their care pathway, from point of referral to treatment, in accordance with the pathway steps and timescales agreed by the BPTP (Best practice timed pathway). Using hospital IT systems of reporting including Etrack, Epro, radiology reporting systems and histology.
- To ensure the patients are tracked through every stage of the patient pathway on a daily basis, to ensure diagnostic tests and any subsequent treatments are achieved within targets and escalate any potential delays in pathways which will result in Cancer wait times breaching as soon as these are identified, in accordance with the breach protocols.
- Ensuring that patients who require administrative transfer to Addenbrookes as a tertiary centre are referred in accordance with the network and BPTP (Best practice timed pathway) agreed timescales, using the appropriate and fully completed SMDT referral proforma.
- Pro-actively coordinating appointments in accordance with the BPTP (Best practice timed pathway) agreed timescales.
- Escalating diagnostic appointments for target patients, (if booked outside the appropriate timescale).
- Identifying delays at all stage in the patient pathway.
- Highlighting any bottlenecks within the process, communicating regularly with team members, and where possible suggesting and implementing solutions.
- Be accountable for ensuring work produced by yourself is of the quality and quantity required.

- Ensuring that patients who require administrative transfer to Addenbrookes as a tertiary centre are referred in accordance with the network and BPTP (Best practice timed pathway) agreed timescales, using the appropriate and fully completed SMDT referral proforma.
- Pro-actively coordinating appointments in accordance with the BPTP (Best practice timed pathway) agreed timescales.
- Escalating diagnostic appointments for target patients, (if booked outside the appropriate timescale).
- Identifying delays at all stage in the patient pathway. Where this is not possible to follow the escalation procedure and network policy for inter-trust transfers, highlighting any relevant issues to appropriate members of the team.
- Highlighting any bottlenecks within the process, communicating regularly with team members, and where possible suggesting and implementing solutions.
- Be accountable for ensuring work produced by yourself is of the quality and quantity required ensuring expected Trust and department standards and deadlines are met.
- Interpret and adapt work processes offering guidance as required.
- Use a wide range of effective communication skills modifying and adapting to a range of situations including the use of persuasion and tact.
