LDA Hospital at Home is a specialist service working with patients up to 18 years old with moderate to severe learning disability and autism. The Thames Valley Provider Collaborative is a network of service providers covering Oxfordshire, Buckinghamshire, Berkshire, Gloucestershire, Swindon, Wiltshire and Bath and North East Somerset (BaNES), working collaboratively to provide high quality care for the most unwell young people as close to home as possible.

We are currently recruiting our multi-disciplinary team (MDT) who will work with young people diagnosed with moderate to severe learning disabilities and autism, who are at risk of admission due to mental health difficulties.

You will be based in Oxford in our new offices located above the meadow unit on the Warneford site.

We are looking for a well-motivated, conscientious and friendly person.

You will have contact with service users, relatives, other members of the public and professionals from partner organisations. You will have an important role in helping the service to run efficiently in a busy and sometimes demanding environment providing an efficient, high quality, confidential administrative and secretarial service to the team.

This role will involve frontline tasks, including dealing with telephone enquiries, booking rooms, making appointments, data entry and using the in-house IT systems.

**Some of the tasks of the role will be**:

- To work autonomously, making independent decisions and organising own workflow.
- To provide administration support to members of team utilising a high level of IT skills to produce quality documentation to support the work of the team using the appropriate Microsoft software and adopting the Trust’s corporate standards and style.
- To assist in the organisation and management of the patient referral and discharge process.
- To assist in the organisation and maintenance of an effective medical/patient records system including patient registration and other data entry into the electronic health records. To manage the electronic management of the team’s caseloads, waiting lists and appointments, to include allocating to and discharging from HCP caseload.
- To type/process correspondence, minutes, documents, reports as necessary.
- Suggest and make recommendations for improvement to existing administration procedures.
- To sort, scan and upload to electronic health records and distribute mail, ensuring matters of priority are brought to the attention of relevant people.
- To work with managers within Operational Services and IT to maintain data quality and make improvements to data collection where necessary.
- To organise and maintain quarterly figures for reporting and assisting the team when required with data for compiling reports.
- To arrange patient appointments, book venues for appointments, courses and meetings.
- To be flexible and provide cross cover to the Provider Collaborative Administration Team/Single Point of Access Team.
- Integrate Hospital at Home administration functions into the Provider Collaborative Single Point of Access (SPA) to ensure continuity of processes.
- To collate staff records regarding training, annual leave and sickness leave etc.
- To maintain stationery stocks and process orders/invoices through e-procurement.
- To work closely with team members, providing a flexible service and being willing to assist staff with a variety of development work as and when required.
