It is recognized that people in detention centres are at greater risk of poorer health than the general population. Proactive encouragement and support to engage with health services can prevent avoidable differences in health outcomes and positively influence the risk of reoffending. Many individuals situated in Yarl's Wood IRC are working through times of change; the psychosocial service aims to provide those with health & wellbeing and substance misuse concerns with the tools to support themselves and promote healthy living in their futures.

The role of the Health & Wellbeing Practitioner is to identify those that need support in a number of areas impacting their general wellbeing. This may range from lifestyle choices, substance misuse concerns to low level mental health factors.

You will be expected to assess risk and need of individuals. Manage a caseload, conduct key working sessions with patients and facilitate group sessions. You will be required to use System One for case notes and management of care and release planning. You will be expected to contribute to multi-disciplinary team meetings with both internal and external stakeholders.

This role is expanding our current provision which allows for flexibility and joint working to create a psychosocial service to meet our current demand within Yarl's Wood IRC.

NHFT is an integrated primary care and mental health Trust, providing physical, mental health and specialty services in both hospital settings and out in the community. Because we put the person at the centre of all we do, we focus on delivering care that is as easy to access as possible. This means many of our services can be provided at home, work or in schools. We also provide health services to various prisons and detention centres in Bedfordshire and Cambridgeshire.
