If the answer is yes, then Changing Futures Stoke-on-Trent has an exciting opportunity for you to join its dynamic team as a Case Co-ordinator.

As Case Co-Ordinator within the Changing Futures Programme you will be working with a small caseload of clients to support their engagement with statutory and voluntary services, ensuring they are able to access the support they require. You will be an advocate for the client, ensuring their thoughts and feelings are heard and adopt a strength based approach.

Working as part of a small team of Case Co-Ordinators you will identify any gaps in the system that cause barriers and work collaboratively with your team and partner agencies to effect system and service change. You will work flexibly and creatively with clients to reduce crises and be responsible for utilising the client's personal budget to support their needs effectively. The Changing Futures programme is a £64 million joint initiative by the Department for Levelling Up, Housing and Communities (DLUHC) and The National Lottery CommunityFund, the largest funder of community activity in the UK.

Changing Futures Stoke-on-Trent works in partnership to better support those who experience multiple disadvantage, providing essential support and treatment for vulnerable adults facing a combination of homelessness, substance misuse, poor mental health, domestic abuse, or contact with the criminal justice system.

The role is fixed term until 31st March 2025.
