An exciting opportunity has arisen for a Band 3 support worker to join the Home-Based Treatment Team (HBT) in Salford.

The post holder will be integral in delivering psychosocial interventions and supported activities in the community to service users, as well as liaising with their carers/families to provide support.

The HBT are committed to supporting and developing support workers and aim to offer additional training and development opportunities to staff within the team.

The post holder will provide Support, give time to an allocated group of service users and promote their recovery. To assist the Multi-disciplinary team to assess, plan, implement and evaluate individual care plans. The purpose of this post is to work as part of a team focusing on the direct needs of service users working across boundaries of care and organisations to promote social inclusion and promote active participation in an individualised recovery plan using the agreed set of values and skills to underpin their day-to-day work

The post holder will be required to work 37.5 hours a week including weekends and bank holidays on a rota basis.
