**Outreach Worker** to join our Leeds Outreach Service. You will be expected to locate, identify, engage and support individuals that are rough sleeping, or at risk of rough sleeping in the Leeds area. Working with rough sleepers to make positive changes in their life and move towards settled accommodation. Amplifying the voice of individuals we work with to express their strengths and needs.

**Hours**:37.5hrs per week

**Salary**:£26,393 - £30,316

**Responsibilities**:
**About the role**:

- Going on outreach shifts; these include early mornings, nights, and weekends. The service covers 7 days
- Engaging with rough sleepers and people involved in street based anti-social behaviour
- Working with partner agencies to support service users to access accommodation, physical and mental health services, substance misuse services, benefits etc
- To work closely with the drug and alcohol teams within the service to ensure that substance misusers and problem drinkers are able to access treatment
- To maintain accurate and comprehensive service user records in line with Change Grow Live's procedures and protocols and the requirements of the local authority, maintaining confidentiality and sharing information with appropriate agencies
- Work flexibly within an agreed number of hours of work to maintain the most appropriate level of service provision. This includes unsociable hours, 7 days a week and working during times of severe weather

**About you**:

- Experience of engaging and providing meaningful support to individuals with complex support needs
- Understanding of the needs of people who are rough sleeping and/or begging and in-depth knowledge of at least one of the following: substance misuse, alcohol misuse, mental health
- Experience of working with people in crisis
- Knowledge of safeguarding procedures applicable to children and vulnerable adults
- Ability to manage individuals with challenging behaviour
- Driving licence and access to own vehicle (desirable but not essential)
