**Care Coordinator Wanted**

We are currently seeking a compassionate and dedicated Care Coordinator in Bloxwich. As a Care Coordinator, you will work within a multidisciplinary environment to provide intensive, short-term support to enable people with physical or mental health needs to lead a fulfilling life. This is a fantastic opportunity to make a difference in the lives of our service users while developing your career in the care industry.

**Key responsibilities**:

- Assess the needs of service users and plan and set up appropriate and personalised packages of support
- Provide person-centred, outcome-focused services to shared objectives ensuring people are offered easy and fast access, choice and control over flexible service options, support in making informed decisions, control over the appropriate risks of support plans, and beneficial changes to reflect changing circumstances and preferences
- Monitor and carry out timely and frequent reviews of people's support plans in compliance with tenant's goal plans
- Liaise with and maintain effective working relationships with other local services, specialist teams, and health colleagues relevant to the needs of the tenants
- Ensure compliance with CQC regulations and quality standards are met

**Requirements**:

- Previous experience in a care or health-related role
- NVQ Level 3 in Health and Social Care or equivalent qualification
- A compassionate and caring nature
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills

**Benefits**:
