As an Emotional Well-being Worker within the Foundry Primary Care Network and contribute to the delivery of excellent patient care To provide one to one support, both emotionally and practically, to a cohort of identified patients with complex mental health and social needs. To communicate effectively with patients, colleagues, and stakeholders both within the Network and externally. To contribute to team and service development and planning To attend and contribute to meeting and multi-disciplinary meetings that support the Networks vision. To work build and maintain good professional relationships both within the Network and externally and effectively collaborate with other agencies and partners.

To be committed to ongoing professional development, including safeguarding and mental capacity training. To work proactively within the team and other teams to ensure integrated multidisciplinary working is successful and embedded throughout the Network. Patient Centred To deliver services that consistently meets patient need To demonstrate a positive and inclusive approach (to customer service) by recognising the diverse needs of patients and barriers they may face To proactively provide tailored practical and emotional support, advice and encouragement to enable patients to remain/become resilient and independent To actively investigate, develop and maintain knowledge of community resources within the locality. People Skills Understands and demonstrates the skills and behaviours required to ensure successful relationships between self and others, including colleagues, patients, clinicians and other stakeholders To display sensitivity and tolerance to individual and cultural differences, diverse opinions and contributions, and listen to alternative views and show consideration for patients, colleagues and partners alike.

To display openness, attentiveness and enthusiasm through verbal and non-verbal behaviour Communication To tailor communication both verbal and written to suit the audience. To communicate effectively with a wide range of people including patients, clinicians, and partners. To ask questions and clarify understanding, checking that communication has been received, understood, interpreted correctly and has the desired effect To ensure that written communication is factual, accurate and timely and clinical system entries are in accordance with the Networks policies. Collaborative working To create and maintain effective open and constructive working relationships with patients, team and colleagues, clinicians and other partners.

To make effective use of professional relationships and networking and share and seek information on issues that may benefit the team or other parts of the Network. To build and maintain knowledge and relationships within the wider community, including the community groups, the VCF sector and the local People Keeping Well partnerships. Attend and contribute to multidisciplinary work both within the Network and externally and build and maintain good professional relationships. Change Management To contribute to and support change to improve and develop services To respond positively to change, seeking clarification and questioning appropriately where necessary.

To be aware of policies in place around safeguarding adults and children, and recognise you and your team will be required to complete an enhanced Disclosure and Barred Service Check To adhere to the Networks policies and procedures, including those relating to health and safety; equalities, diversity and inclusion; safeguarding procedures; financial instructions; procurement and GDPR To undertake any other duties in agreement with the post holder and manager.
