The post holder will be responsible for providing advice, support and delivery of public health interventions for the Healthy Families Service. They will contribute to the collaborative development of evidence based interventions to improve the health and wellbeing of children, young people and their families. They will work as a member of Healthy Families Team delivering support, training and patient care across multiple health improvement programmes for children, young people and their families. The post holder will be responsible for supervising the work of the Health Families Assistant Practitioners within their public health work stream. The post holder will be responsible for reporting outcomes and performance to the Healthy Families Team Leader.
- Responsible for ensuring the service delivers high quality care to children, young people and their families by ensuring;
- Evidence based pathways are in place to help improve health & wellbeing.
- Evidence based initiatives are established that promote a healthier lifestyle.
- System are in place to build capacity in individuals and groups to promote self-help.
- Evidence based training is offered to 0-19/25 health and care workforce, Early Years Settings, Schools and Colleges on health improvement topics/RSE.
- Coordination and delivery of RSE within schools and colleges
