This is an excellent opportunity to join our Community Specialist Palliative Care team for anyone who has an interest in supporting and improving the quality of life for patients with life limiting illnesses. Whether you are an experienced Band 4 wanting a change or a Therapy assistant wanting to develop their skills working in a specialist area.
The post is part of new funding to develop the specialist palliative care team working alongside the Occupational Therapist as well as the nurses, dietitian, and specialty doctor in the team to provide integrated care. The team is friendly and welcoming and warmly supports colleagues. Based at The Friarage Hospital, Northallerton and working across Hambleton and Richmondshire seeing patients in their own home.
The post holder will be expected to have experience working as an assistant with OT or Physio in the NHS and an understanding of palliative care.
As well as completing joint visits with therapists the post holder will be expected to hold their own caseload, undertaking independent assessments and interventions to provide support with life story work, engagement in meaningful activity, fatigue/breathlessness management, ADL practice, education and equipment provision with the aim of improving quality of life and function for each patient.
The ability to travel independently is an essential part of this post as you will work across Hambleton and Richmondshire community providing a service within patients own homes.
You will work closely with the acute therapy team based at The Friarage Hospital as well as community therapists and nearby hospice services to provide a seamless pathway for palliative patients and improved patient outcomes.
South Tees Hospitals NHS Foundation Trust is one of the largest teaching hospital trusts in the country, close to the North Yorkshire Moors and within easy reach of Newcastle and Leeds. It comprises two acute hospitals, James Cook University Hospital and Friarage Hospital Northallerton and a number of community hospitals and community services. Therapy services at South Tees NHS trust are also part of the wider Integrated Therapy service in Middlesbrough, Redcar & Cleveland.
This post offers development in clinical and leadership skills and the post holder will be supported to attend relevant courses. We have an established and comprehensive training program and our staff are encouraged and supported in their CPD through workshops, clinical supervision and the appraisal process.
