Lead Nurse Children in Care and Youth Justice

Key Responsibilities Provide clinical service for Children in Care including Unaccompanied Asylum Seeking Children/ young people, children with complex health needs and Care Leavers. To line manager the Youth Justice Service Nurse. Assist Mid Yorkshire Hospital Trust in fulfilling its responsibilities as a provider service, working alongside commissioners to improve health outcomes for Children in Care across the District and within a 50 mile radius. Provide information as requested, in verbal and written formats as appropriate.
Ensure appropriate mechanisms are in place to promote and sustain effective working relationships between staff and partner agencies in developing, promoting and addressing the health needs of Children in Care. Workingwithin multi-agency processes to identify potential risk of harm to children in our care. Provide support and advice to all relevant staff within the trust and other agencies in relation to the health needs of Children in Care and Young People. To provide specialist advice, supervision and support to your defined caseload and team.
Provide support and supervision as required to staff in the team or students on placement. To undertake Health Needs Assessments and therapeutic interventions with CIC & young People, and co-ordinate Health recommendations. Be responsible for auditing the completed health assessment process and ensuring that each child or young person has a current, up to date health plan in line with statutory requirements. Feed back to staff on their training needs with regard to completing health assessments.
Work directly with Children in Care, Unaccompanied Asylum Seeking Children/ young people and Care Leavers who cannot/do not engage with mainstream services to provide advice and support on issues relating to health and wellbeing. Attend appropriate strategy meetings and Team around the Child and planning meetings to provide a health perspective and assessment of risk to the child or young person and ensure multi-agency working is adhered to. Contribute to the development of a district wide database for Children in Care through submission of accurate and timely information, and contribute to achieving health assessments for this group of children. Be able to identify unmet health needs / gaps in service provision and promote innovative ideas to meeting those needs.
Keep self-updated regarding all issues concerning safeguarding, including legal, research and government policy in order to ensure that advice given to professionals follows current best practice. Adhere to NMC Code of Professional conduct and be conversant with the scope of professional practice. Work within national and local guidelines. Ensure that all care is research based and delivered to an agreed standard, working with appropriate evaluation processes.
Be willing to participate in research and audit and disseminate findings across the district in order to influence future development of the service As a Band 7 Lead nurse, you will be expected to professionally lead the team. As a band 7 Nurse you will be responsible for supporting and leading identified and agreed aspects of the service, in the development of care systems, protocols and guidelines working as part of a multi-disciplinary team and developing excellent interdepartmental relationships with excellent communication with a focus on quality improvement.
