Location: Mercian Close Erewash
Area of work: Across Erewash and South Derbyshire Areas
**As an Enablement Worker, your key responsibilities include**:
To provide a wide range of practical, social, and general care services for individuals using the enablement service, in line with the agreed Personal Service Plan.
Alert managers to changes in presenting needs and respond in emergency situations liaising with colleagues, other agencies, and carers as appropriate.
Support and encourage individuals to maximise independence through engagement in social activity, working with local community networks, voluntary and self-help groups.
Complete records in respect of, Personal Service Plans case notes.
Liaise with colleagues, voluntary bodies, health service colleagues and other agencies, working with people with a range of complex needs and their families/carers, ensuring the safeguarding of vulnerable adults.
Maintain confidentiality and adhere to the departments recording systems.
**Additional Highlights**:
️ Travel Required: Explore the Erewash and South Derbyshire area and occasional trips to other Derbyshire locations for training and meetings.
Diverse Work: Engage with working age adults on short-term and long-term support packages.

The job may require unsocial hours working including weekends and Bank Holidays. Payment arrangements will be detailed in any contract offer made.
Saturday and Sunday hours will be paid at plain time where it is part of your normal working pattern unless it is between 7pm and 7am, where enhanced rates of pay apply. Enhanced pay rates are paid for Bank Holiday working.
