We are looking for an experienced person with a background of working with the elderly in a care setting. The role is office based and will be based at one of our homes in Dartford, Kent.

**The Role**

This role is vital to ensure that referrals and assessments are processed correctly and within the time scales set out in our service specifications. You will be contacting and liaising with external referrers, the local authority, NHS, Privately Funded, and other community organisations. They will be disclosing very personal information and therefore it is essential that we are committed to safeguarding their confidentiality and therefore it is vital that you adhere to all relevant legislation, policies and procedures including confidentiality and GDPR.

You will also be supporting operational teams to maximize occupancy.

This role involves the following duties and responsibilities
- Process all referrals into RCB Healthcare Group
- Arrange and conduct initial ‘pre-assessment’ calls
- Coordinate the allocation of new residents to empty rooms
- Build positive relationships with Local Authorities/NHS, current and prospective referrers to increase the volume of referrals to RCB Healthcare
- Advise referrers on the referral criteria and process
- Update referrals, occupancy and empty rooms logs, and communicate clearly and promptly with RCB Healthcare Management.
- Provide support and advice to Home Managers regarding move on options and funding options to help with move on.
- Stay abreast of funding options.
- Maintaining Occupancy.
- Any other duties required by the organisation

- Strong independent working skills, and ability to work within a team
